バーバラ・キシレフスキー博士は、看護師資格も有する発達心理学者で、クイーンズ大学で教鞭をとるとともに、隣接するキングストン総合病院の小児科、産婦人科と連携し、人間の胎児期および新生児期の感覚の発達に焦点をあてた基礎研究と応用研究を推進してきました。主要なものの第一は、低リスク胎児を対象とした研究で、ホワイトノイズや音楽などの音刺激や振動音響刺激に対する心拍や胎動を検出し、発達的変化を検討です。これらを通じて、胎児期から新生児への反応の連続性を検証するとともに、母親の声を識別するなど、胎児が胎外からの環境音の特性に注意し、学習し、記憶することを明らかにしてきました。第二は、母親の糖尿病や高血圧、喫煙、未熟児など、高リスク胎児を対象とした研究で、これらの胎児の聴覚刺激への異常な探索反応パターンを同定してきました。このたび、日本学術振興会外国人招へい研究者として来日されましたので、以下の内容でこれまでの研究をご紹介いただきます。
[発表要旨]
Placental insufficiency results in decreased nutrition and oxygen supply to the fetus and is one of the most common causes of fetal/newborn growth restriction. It is important because of the long term effects on the offspring, including neurodevelopmental deficits. Over 20 years of follow-up of growth restricted infants has shown an increased risk of language deficits by 2 to 9 years of age, learning disabilities by 9-11 years of age, and subsequent behaviour problems. Thus, we hypothesize that atypical fetal auditory processing occurs in the presence of placental insufficiency and affects later language development. Because fetal behaviour reflects brain development, to test our hypothesis we have been examining sound induced fetal behaviours. Using short bursts of relatively loud sound, we established that fetuses hear by about 29 weeks GA. Using longer duration relatively lower sound intensities, we have been comparing auditory information processing in fetuses in high-risk pregnancies complicated by conditions which are associated with placental insufficiency with that of low-risk fetuses delivering as healthy, full-term newborns. We have found atypical fetal auditory processing in pregnancies complicated by maternal hypertension, smoking and fetal growth restriction, providing evidence to support our hypothesis. We conclude that environmental sounds are available or shaping neural networks and laying the foundation for language learning before birth. Fetuses in pregnancies complicated by conditions associated with placental insufficiency show atypical auditory processing. Whether the effects are temporary or permanent changes in function is unknown.
